• Single ECG sensitivity: only ~50% for ACS
• Single troponin can miss an evolving MI

Half of ACS patients have a normal first ECG.

Mandatory serial testing:
• Repeat ECG in 15-30 min if non-diagnostic
• Repeat troponin at 2-3h (or hs-troponin 0/1h protocol)

One test is never enough.
