Before reperfusion: <185/110 or no tPA
Post-reperfusion: tPA → 130-140 | EVT → 140-180

Spike = Stop and scan (possible hemorrhage)
Avoid extremes — especially <140 post-EVT
Frequent monitoring (15-30-60 rule)
Elevated OK if no reperfusion (permissive hypertension)

Post-EVT is NOT the same as post-tPA!
