A — Assess: FAST-R in 15 minutes — Fall history, Assessment + judgment, Screen meds, Test mobility, Respond individually
I — Intervene: Target the Intrinsic Five (prior falls, weakness, balance, cognition, orthostatic hypotension), review FRIDs, PT referral
R — Repeat: Reassess when status changes, track mobility at each encounter, never accept "mechanical fall"
