Myth: "The tool says low risk — no additional measures needed"

Reality: Fall risk tools have an AUC of 0.5-0.7, barely better than chance

Margaret scored "low risk" this morning. Four hours later: fractured hip on the floor of room 312.

A "low risk" score means the tool couldn't detect her specific risk factors. It does NOT mean she won't fall.
