Before discharge or downgrade, check ALL four:

S — Shock signs? (hypotension, tachycardia, poor perfusion)
T — Tenderness out of proportion to exam?
O — Older than 65?
P — Peritoneal signs? (guarding, rebound, rigidity)

If ANY element is present → Escalate. Do not discharge.
