Before ANY other diagnosis, exclude all 6:

A — ACS (pressure, radiation, diaphoresis)
P — PE (pleuritic pain, dyspnea, DVT risk)
E — Esophageal rupture (post-vomiting, subQ air)
T — Tamponade (Beck's triad, pulsus paradoxus)
T — Tension pneumothorax (absent breath sounds)
C — aCute aortic dissection (tearing, BP diff >20)
