High-risk drug classes:
• Diuretics (furosemide, HCTZ) → volume depletion
• Alpha-blockers (prazosin, tamsulosin) → vasodilation
• Vasodilators (hydralazine, CCBs) → direct vascular effect
• Antidepressants (TCAs, SSRIs) → autonomic effects
• Antipsychotics → alpha-blockade
• Opioids → vasodilation
Rule: More BP medications = higher OH risk. Always review the med list.
