🚨 Altered mentation is the most commonly MISSED qSOFA criterion

"That's her baseline" — Is it? Any CHANGE from baseline counts.
"He's just tired" — New lethargy = altered mentation.
"She's sundowning" — Sundowning + infection = qSOFA positive.
"He's 85, of course he's confused" — Age doesn't explain acute changes.

Rule: Infection + any mental status change = count it. Always.
